New Patient Information

Things to know for your first appointment:

  • In consideration of our asthma patients, we request that all patients and visitors not wear perfume or cologne.
  • Please be prepared to list your medications and recent problems. Recent x-ray dates and locations are helpful.
  • No antihistamines 7 days prior to appointment (unless you have hives).

  • Stay on your asthma medications.
  • Bring your insurance card(s).
  • Bring your copay if you have one.
  • Please call your primary physician to verify a referral has been sent to our office by your insurance company, if required. (Most HMO's require a referral. If you do not have one, you may have to reschedule your appointment after one has been received by our office.)
  • Please allow two hours for your appointment to allow for a full evaluation and possible testing.

If you have any questions or concerns, please feel free to call us during office hours.

Please note: Failure to cancel with a 24-hour notice will incure a no-show fee.
